Establish care by contacting us to see if we service a specific nursing facility or assisted living site. If you are seeking to establish care at our clinic, Achieve Health or set up a New Patient Telehealth Visit contact us here. Help the staff get to know you by reviewing your medications, past medical history, allergies and past surgical history. You can upload this information via our portal or speaking directly with the nurse. During this initial visit labs may be ordered, records requested and a foundation for your health and well being will be discussed.
